You don't need an agency to apply. Here's exactly how it works — and we'll help you through it if you want.
Anyone can start this: the person who needs help, a family member, a friend, a doctor's office, or a hospital social worker. Contact the MDHHS office in the county where the person lives — by phone, mail, fax, or in person — and say you want to request Home Help services.
An Adult Services Worker is assigned and sends form DHS-390, the Adult Services Application, which the person needing care completes and signs.
A physician, nurse practitioner, physician assistant, occupational therapist, or physical therapist completes form MDHHS-6200, the Adult Services Medical Needs Certification.
They come to the home, review daily activities, and determine how many monthly hours are approved.
Once approved, the choice is yours. Your caseworker cannot recommend a specific agency — that decision belongs to you. If you choose Alamat, tell your caseworker, and we handle the rest.
MDHHS assigns you to an office based on where the person needing care lives. If you reach the wrong one, they'll point you to the right office. Below are the district offices for the counties we serve most often.
